Q: do they have a slot or wing to insert them into a holder?

I am uncertain of the terminology but I would call it a wing that inserts into a credit card slot.

Q: 3 of the 4 photos for these show them joined on the long side, but 1 photo shows them joined on the short side. Can anyone verify which they are?

YOU Can select different sizes and configurations in the small boxes to the right and below the thumbprint photos. In the photos they sometimes show different models but not the very first one. So click on first then hover and or click on the selection boxes one by one to see the item in the larger thumbprint change. SO that's why you may have seen 3 different designs. But clearly if you are on the first main thumbprint selected then select for certain this size for trifold bilfolds some bifolds too and then DOUBLE READ the title to make sure all is in agreement and correctly chosen then and only then is it safe for you to click add or buy now. I'm sure in 2 to 3 years of buying now on Amazon you've caught on to this but just in case someone else is new to the game I thought I would explain it. Good luck and stay safe.
